Cardiac Rehabilitation Programme

"Cardiac Rehabilitation has been defined by the World Health Organisation (WHO) as “the sum of activities required to influence favourably the underlying cause of the disease, as well as to ensure the patients the best possible physical, mental and social conditions so that they may by their own efforts preserve, or resume when lost, as normal a place as possible in the life of the community.  Rehabilitation cannot be regarded as an isolated form of therapy but must be integrated with the whole treatment, of which it forms only one facet.”

 

Introduction

Cardiac Rehabilitation is a multi-disciplinary service involved in the education of patients regarding diagnosis, treatment, recovery and identification of their risk factors. Its’ main aim is to slow, stop or reverse the progression of heart disease by promoting a healthy lifestyle.

Four Phases to Cardiac Rehabilitation

  • Phase One: In-patient stay
  • Phase Two: The immediate (2-6 weeks) post-discharge period
  • Phase Three: The intermediate (2-6 months) post-discharge period
  • Phase Four: Community based service – post Phase Three
