Purpose of the Post
Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008.
Caseload
e.g. Patients – adults over 16 years of age
· Clinical Nurse Specialist (Cardiac Rehabilitation) will focus initially on the following patient groups on patients over the age of 16 years with a diagnosis of myocardial infarction, angina, heart failure, post coronary artery bypass or valve surgery, post angioplasty and stent insertion, post cardiac transplantation, post cardiac device insertion (ICD, CRT, pacemaker) according to scope of practice.
