GP & Clinican Information

 

Cork University Hospital has two distinct referral pathways to the wound care service: Inpatient and Outpatient.  

 

In-Patient Referrals 

In-Patient referrals are accepted from the following sources: 

  • Nursing
  • Medical Teams
  • Surgical Teams
  • Allied Health Professionals. 

Referrals will be accepted by letter, email or by phone. Phone referrals must be followed up with the completion of the Acrobat Reader logo  RANP In-Patient Referral Form

 

Out Patient Referrals 

Out-Patient referrals are accepted from the following sources: 

  • General Practitioners (GPs) and Practice Nurses  
  • Public Health Nurses
  • Nursing Homes/Community Hospitals. 

Referrals will be accepted by letter, email or by phone. Phone referrals must be followed up with the completion of the Acrobat Reader logo RANP Out-Patient Referral Form
